My name is Traci. I previously had my own Company for over 10 years as a Geriatric Care Coordinator I also specialize in Alzheimer's disease and other types of dementia. Care Coordinators focus on your loved ones person centered individualized tailored needs. I do everything you as the family are not able to do. due to careers or living out of town. My job ensures your peace of mind and your loved ones safety and overall health and well being.. I look forward to meeting you all and start learning what is needed of me to start giving your loved one all the love and care they need. I will be that person who drops in unannounced to ensure your loved ones treatment is being done at the highest standard. I will listen to client and family about things the client likes to do. I will take care of all appointments and follow up appointments. I will create a person centered daily care plan. I will train caregivers on how you want care delivered and much more big or small. I will LOVE them.
