About Tamara

I am Alzheimer's/Dementia certified. My Goal is to help families keep their loved one at home instead of in Assisted Living or Memory care. My goal is to provide personalized patient care and provide a care plan the entire family can follow. I understand the difficulties that arise to manage caring for a loved one, while raising a family and sustaining a career. Care coordination and caring for loved one can be a daunting task and usually requires time off of work to arrange all of the appointments. I help families care for loved ones & navigate the complexity of the health care system. I have worked in multiple healthcare settings hospital, nursing home and home care.
